Abstract:
Tuberculosis (TB) is a global health problem with an estimated 2 million deaths per year. There is an urgent need for interventions to prevent infection after exposure to M. tuberculosis, and progression from latent TB infection (LTBI) to active TB. We hypothesize that vitamin D deficiency is associated with increased risk of TB infection and progression to active TB, through autophagy and altered M.tb induced cytokine responses.
In this study we will include patients with active TB, and screen their household for LTBI. We will then examine their vitamin D status, and genetic polymorphisms in the vitamin D immune signaling pathway and autophagy. In a subgroup we will examine in-vitro immune response to M.tb, using whole blood cytokine assays, in relation to vitamin D status and relevant genetic polymorphisms. Finally, using multivariable logistic regression, vitamin D deficiency, and haplotypes for VDR and downstream signaling and effector molecules will be correlated with active TB and LTBI, taking known risk factors for TB into account. Patient specimens including DNA and microbioma will be archived.
This study, with a highly competent research team, combines careful field epidemiology with cutting edge genetics, immunology and systems biology. It will generate unique data and strengthen TB control in Romania through household screening. It may provide biological basis for vitamin D supplementation in LTBI and/or active TB.
